Saint Patrick School is currently accepting registrations for the 2008/2009 school year. If you have not yet registered your child, or if you
would like to learn more about Saint Patrick School, please call the
school office at 978-683-5822.

Please bring the following documents with you when you register:
Requirements
- Birth Certificate
- Baptismal Certificate (even if child was baptized at St. Patrick Church)
- Immunization Record
- $50 Registration Fee (Non Refundable)
- Physical (within one year of entry)
- Vaccinations, Lead Screening, Tuberculin Screening
Massachusetts Department of Public Health Optimal Requirements
DPT – 5 doses Polio – 4 doses MMR – 2 doses
Hepatitis B Vaccine – 3 doses Lead Screening Test
1 dose of Varicella (Chicken Pox) or verification of having had the disease
Tuberculin Test (test or screening)
Please include a copy of your child’s immunization history and last physical examination with your completed registration papers.
